Muscle loss on GLP-1 medication

Losing weight quickly on a GLP-1 takes some weight from lean tissue as well as fat. Published trials have reported lean mass making up a wide range of total weight lost, often cited somewhere between a quarter and 40%, though estimates vary a lot with the study, the measurement method and the person. This is not unique to GLP-1s; it happens with rapid weight loss generally. The two things most consistently linked to keeping more muscle are eating enough protein and doing resistance training throughout.

Why it happens

When you are in a sustained calorie deficit, your body draws on stored energy, and it does not draw exclusively on fat. Some lean tissue goes too. Two things about GLP-1 treatment make that worth paying attention to: the deficit is often large, because appetite suppression is strong; and protein intake tends to fall along with everything else, precisely when it most needs not to.

It is worth being clear that this is a feature of fast weight loss, not a special property of the medication. Bariatric surgery and very low-calorie diets show the same pattern.

What the research actually says

Reported figures vary widely, and anyone quoting you a single confident percentage is overselling the evidence. Different trials use different methods (DEXA, bioimpedance, MRI) over different durations, in different populations. What is reasonably consistent across them is the direction: a meaningful share of the weight lost is lean tissue, and that share is smaller in people eating more protein and doing resistance work.

Why it matters beyond appearance

The two levers that actually move

1. Protein, every day, including the bad days

This is the one that fails on a GLP-1, and it fails for an obvious reason: on the two or three days after your injection you may not want to eat anything at all. A protein target you hit four days a week and miss three is the single most common pattern we see. Our protein guide covers how to work out a number and how to hit it on a small appetite.

2. Resistance training, scaled to the day

Lifting during a deficit is repeatedly reported to preserve more lean mass than dieting alone or dieting with cardio. The practical problem is that a fixed programme collides with a week where two days have no energy in them. Scaling the week (hard sessions on your return days, nothing required on your worst suppression day) is far more sustainable than a plan you abandon in week three. See training on a GLP-1.

Things that help around the edges

How to tell whether it is happening to you

The scale cannot tell you. What can: tape measurements at the arm and thigh over time, and whether ordinary physical tasks feel easier or harder than they did a month ago. Recompose builds both into a weekly check-in so the trend is visible rather than guessed at. If everyday strength is clearly declining, that is worth raising with your GP or pharmacist.

Common questions

How much muscle do you lose on Mounjaro or Wegovy?

Estimates vary widely between studies and individuals. Trials have reported lean mass making up a substantial share of total weight lost (figures between roughly a quarter and 40% are commonly cited) but the measurement methods and populations differ enough that no single number is reliable. Higher protein intake and resistance training are consistently associated with keeping more of it.

Is muscle loss on a GLP-1 permanent?

Muscle can be rebuilt with resistance training and adequate protein, though it is slower to regain than to lose, particularly with age. Preventing the loss in the first place is considerably easier than reversing it.

Can I avoid muscle loss completely?

Realistically, no. Some lean tissue is lost with any significant weight loss. The aim is to keep as much as possible, not to lose none, and protein plus resistance training is what the evidence points at.

Should I stop my medication if I am worried about muscle loss?

That is a decision for the prescriber who manages your treatment, and not one to make on the basis of an article. Raise the concern with them. There are usually things to adjust in how you eat and train long before the medication is the question.
